1.

 Completed Uzbekistan Visa Application
  Complete Uzbekistan Visa Application Online - website will open in a new window (at the bottom of the page click CLICK CONTINUE WITH ONLINE APPLICATION) ). As of September 8, 2009, all visa applicants will be required to fill out on-line the Uzbekistan Visa Application Form. The information that you enter is not submitted electronically. It is only used to generate the printed form and your request number. After completing the application form online, please print it. Please do not forget to sign and date your application. Note:You must use Internet Explorer and Microsoft Windows OS to complete online application. If you use MAC or any other browser (e.g. Safari, Firefox, Netscape), it may not work properly!

 Valid U.S./Non-US Passport
  You have to MAIL us your current valid and signed US/non-us passport. Passport should be valid at least SIX (6) months after your intended arrival to Uzbekistan. Also, you should make sure you have at least ONE completely blank page for visas in your passport. Amendment pages (pages 22, 23 and 24) do not count.
If your passport is about to expire or needs more pages, please obtain new, renew or add pages to Your passport.

3.

 ONE (1) Passport-Style Photograph
  It must be 2x2 inches in size. We always recommend using a professional photographer to avoid delays.You can choose to submit digitized photo to us via email: orders@travelvisapro.com (in subject line put travelers first and last names). Additional $10 Photo Printing FEE will be added to your request. NOTE: Photo must be high resolution, clear, and provide proper contrast and white background for it to be accepted by the consulate.

 Non-US Passport Holders (proof of your legal status in the USA)
  Please provide a proof of your legal status in the USA (i.e. copy of your green card (front and back), valid visa, I-20, IAP-66, etc.)

Additional Uzbekistan visa information:
NON-US CITIZENS ONLY:Tourist Vouchers from the National Company "UZBEKTOURISM" or authorized travel agencies are required (except for Citizens of Austria, Belgium, France, Germany, Great Britain, Italy, Japan, Spain, Switzerland) for the issuance of tourist visas. Please call if you do not have your own VISA INVITATION. We will obtain one for an additional fee of $100 and it will take 10 business days.

FOR BUSINESS VISAS ONLY: Business multiple or single-entry visas are issued only upon the confirmation received by the Embassy from the Ministry of Foreign Affairs of Uzbekistan. IF YOU DON'T HAVE ONE, please obtain it.

NON-US PASSPORT HOLDERS: If you're non-US passport holder, you'll pay different consular fee. Please call 888-470-8472for pricing.
